The agencies participating in the CPPJ Employee Fitness Program and the MONTHLY FITNESS FEES are as follows:

· YMCA: Adult Fitness - $34.40 Family Fitness - $44.00
Adult General - $25.00 Family General - $36.80
· DYNAMIC DIMENSIONS:
Individual - $25.00 Family - $50.00
* Required 12-month contract.
· CURVES FOR WOMEN: Fees listed are for a 6-month commitment.
Individual - $29.00 +1 Daughter - $39.00
No Initial Membership Fee +2 or More Daughters - $49.00
· LA FITNESS & AEROBICS:
Individual - $25.00 + tax = $27.13 Couple or Family - $35.00 + tax = $37.98
· OUR LADY QUEEN OF HEAVEN FAMILY LIFE COMMUNITY CENTER:
Family - $25.00 (Contributors of record donating $200 or more to OLQH)
Family - $45.00 (Contributors of record donating less than $200 to OLQH)
Provided the employee attends their selected fitness program six (6) times per month, the Calcasieu Parish Police Jury will pay $25.00 towards a single membership and $50.00 towards a family membership, if the spouse is covered through the CPPJ health insurance plan.
Attendance will be monitored, and if an employee does not attend, he/she will lose the Parish contribution benefit and will be responsible for the full monthly fee.
The employee’s share of the monthly fees are payroll deducted bi-weekly.
